Need a Driving Record?
Driving Records may be obtained in two ways:
- You may obtain your Driving Record (DR) via the Internet Driving Record Process. Click Driving Records under Online Services - Click @ DMV.
- To obtain your Driving Record (DR) by mail, follow the steps below.
For Law Enforcement and Court Personnel Only:
The Official Record of Convictions for Violations of Motor Vehicle Laws and Departmental Action Form DL-49 is for use EXCLUSIVELY by law enforcement agencies or the members of the court system. It is used to request Driving Records and suspension letters for prosecution purposes. The DL-49 Requisition Form is used to request a re-supply of the Form DL-49. In order to obtain a Driving Record (DR) you will need to complete:
The DPPA forms are now interactive and can be filled out online BEFORE printing the form.
You may also obtain a DPPA-1 form by contacting a Customer Service Representative at
919-715-7000 or by visiting your local Driver License Office.
Please allow 10 business days processing time when requesting a driving record, this does not include US Postal Service delivery time to or from the DMV.

The fees for a Driving Record (DR) are as follows:

Limited Extract and Complete Extract copies are normally used for insurance and employment purposes. A Certified True copy is usually required for court appearances.

Below are the most common reasons that a request cannot be processed and will be returned
for correction.
- The check or money order was not signed.
- The check or money order has been altered.
- The check or money order was not made payable to "NCDMV".
- Incorrect request form was submitted.
- The requestor did not sign the request document.
- Failure to circle a qualifying exemption on the front of the DL-DPPA-1
- The fee was not received or the fee sent was an incorrect amount.
- Insufficient information (Important: enter all information requested on the form)
- Authorization to release personal information was not obtained on a DL-DPPA-2 (this is required by law if you request another person's record and you do not meet one of the 12 qualifying exemptions on the front of the DL-DPPA-1).
