I Need Homebound Services for a State ID

1. Fill out your request

First, you'll need to complete the Homebound Service Request form and a letter from a physician certifying that you are homebound.​

Once the form is complete, you can send the form​:

• Online to NCDMV​. Please include medical documentation. 
  
• Via fax to (919) 615-7289.

2. NCDMV visit

Once your request is received and processed, an NCDMV customer service representative will reach out to arrange a visit. ​

These are the documents you will need to have for the representative: ​

• One document verifying age and identity
• A Social Security card or one document proving they have a Social Security number
• For U.S. citizens, one document proving residency 
  
• For non-U.S. citizens, one document issued by the U.S. government indicating legal presence
  

• Any document required ​in the fee section ​of the primary s​tate ID page if it applies to you
