NEVI RFP 2 Application Submission Process

​​This webpage explains how to apply for the N.C. Department of Transportation's NEVI Program Round 2 RFP using the required online forms. For help with the forms, email NCNEVI@ncdot.gov with the subject line "Form Assistance."

​A. How to submit applications ​

These applications use JotForm as a platform.  This platform allows users to save and edit forms prior to submission (see Submittal process below). ​

There are two applications that must be submitted: 

  1. Team Information JotForm Link​  – one form for each unique applicant team 
  2. Site Information JotForm Link  – one form for each proposed site
  3. Additional Actions for Applications​ - append addendum, reopen/withdraw application​

Notes on applications: 

  • An applicant team may propose more than one site, but only one site per cluster. 
  • Applicants must complete all required sections of the forms. 
  • Applicants must provide the necessary information/documents directly within the form.
  • Links to information outside the forms will not be reviewed. 

B. Required attachments & documents used in applications​

NCDOT has a c​hecklist of RFP Attachments and D​ocuments webpage that are required for the applications. The list specifies whether the document is an attachment to the RFP, the file format and whether it needs a signature/notary.  

NCDOT recommends having all the attachments and documents ready for upload prior to filling out the form.    

  • The contents of attachments/documents must meet the requirements in Section 5.2, Application Contents, and the reference table.
  • Applicants must ensure certain attachments are signed/notarized prior to uploading. 
  • File sizes are limited to 10MB per file.  
C. How to Submit the JotForm
  • Jotform allows applicants to save and edit applications prior to submitting. There are separate buttons for save and submit.
  • Upon saving the form, applicants will receive an email from JotForm with a link to edit the application. 
  • Upon submitting the form, Applicants will receive an email from JotForm acknowledging receipt of the application. Once the submit button is clicked, you will no longer be able to edit the form. 
  • After you submit a form, you will receive an email from Jotform acknowledging your submission. This email will also contain two identifiers which will be used by NCDOT to identify and link your team/site applicaitons: 
    • ​Applicant ID
    • Submission ID 
D. How to reopen/withdraw an application or attach an addendum acknowledgement

NCDOT has created a third Jotform to handle additional tasks related to your application forms at re-open or withdraw​ Jotform Application(s) webpage​.

  • An applicant can upload signed Addendum Acknowledgement form(s).  
  • An applicant may reopen or withdraw Jotforms after submission by filling out a request at the above Jotform.  ​
