  1. Attachment 3 - Signed Technical Application Form (Searchable PDF format, 30 pages) *
  2. Zip file containing the following:
    1. Organizational chart
    2. Resumes for key personnel (up to 5)
    3. EVSE specifications, diagrams and cut sheets
    4. EVSE connection diagram
    5. Preliminary site design and layout
    6. Area map
  3. Attachment 4 - Signed Cost Proposal Form (Excel format) *
  4. Letter from the Site Host Owner - Signed (Searchable PDF format) *
  5. Attachment 6 - Acknowledgement of FHWA Form 1273 - Signed (Searchable PDF format) *
  6. Acknowledgement of all issued addendums (Format to be detailed in addendum) *
  7. Additional Optional Attachments:
    1. Letter from bank or financial institution (optional)
    2. Attachment 8 - Utility Data Request Form or letter from utility (optional)
  • The maximum number of files is 10 and total size limit is 50MB.
  • You can upload the following file types:

  • Excel spreadsheets (.xlsx)
  • Zip files (.zip)
  • Pictures (.jpg, .png)
    • IMPORTANT:
    1. To assist in this process, NCDOT recommends storing all of your files in one folder.
    2. Choose ALL files to be attached at the same time.
