Proving Social Security

​​​​​​​The N.C. Division of Motor Vehicles recognizes certain documents as proof that an individual has a Social Security number.

The documents below are acceptable proof of your Social Security number for a REAL ID. However, with the exception of a Social Security card, they cannot be used as​ pro​of of identity​.​​

​​​From the Social Security Administration: Social Security card​ (Not allowed: photocopies, laminated cards and metal or plastic replicas)
​From the IRS: 1099 tax form with name and FULL social security number (no other IRS form is acceptable)
​From your employer: W-2 form with name and FULL Social Security number
​From your employer: Payroll record with name and FULL Social Security number

The documents below can prove your Soc​​ial Security number, but cannot be used to obtain a REAL ID or as proof of identity.

From the Department of Health and Human Services: Medicare card reflecting Social Security ​number
​​From the Department of Defense: DD-214 form
​From the military: Military record reflecting Social Security number
